To genuinely cater the specialized needs of hospice patients is what we always thrive to do.

A Basic Understanding of Hospice

In recent years, so many people have been touched by Hospice, whether it be for someone that was directly related, or for someone that we knew as, or through, an acquaintance.

Hospice provides comfort, pain, symptom management, spiritual and social support. This special care treats the family unit, not just the patient, and provides the support and care needed during this difficult and sensitive time.

Hospice Care in Avon, Indiana comes to you. Whether you live in your own home, the home of a loved one, or in a facility, Life’s Journey will be there.

Bereavement care and support are also provided to surviving family members for at least 12 months after hospice care ends.

Services Offered

Hospice is a Medicare Part A covered benefit, along with most insurance plans. It covers all direct
and indirect care provided to the patient. Hospice includes home visits by nursing staff, chaplains, social services, and volunteers. Necessary medical equipment, supplies, and medications related to your diagnosis, all at your home. Our team, along with you, will determine a complete plan of care to meet your goals. We want you to be in charge, but we will be there to offer expert advice.

When Should I Consider Hospice?

The benefits of receiving Hospice care increase if you elect the service sooner rather than later. Stress can be reduced, family members can have a chance to adjust to the situation and more support can be provided. If you believe Hospice may be right for you or a loved one, please don’t hesitate to have the conversation with your physician. For additional Information, please call our office at 317-316-3937.

What is the Cost?

Life’s Journey’s Hospice Provider agreement assures that we will meet the highest standards of care. Our agreement spells out the details of what we provide, as well as what’s expected from the Hospice, according to the required Conditions of Participation. We bill the Hospice Provider and the Hospice Provider bills the Intermediary or the patient. Our Agreement also spells out the cost negotiated with the Hospice Provider for a daily rate.

The cost of Hospice is covered 100% by Medicare with no co-pays or deductibles. This includes all medications, necessary equipment, direct care and support staff to take care of all your Hospice needs that relate to your primary diagnosis. In addition, most insurance policies have some sort of Hospice benefit that has little or no out of pocket cost. Please contact us to help you determine your eligibility.
